Or is it due to the pent up anguish in their hearts from practicing denial, refusing to grow, tell the truth and move on. Dentists are taught a whole bunch of hogwash while in dental schools. it doesn't take much more than an inquiring mind and Google for them to see that their education is seriously deficient. But how may practice denial and stick with the hogwash. Personally I think it is the conflict, like the autopilot HAl in the movie 2001, and mercury that causes so many to self destruct.

It is not a perfect world. I retired from clinical practice at the turn of the century and so I may not know all of the new materials but there are many satisfactory ones. I used a lot of gold and composite with no fluoride or Bis-GMA. Not everyone can tolerate everything but selection is possible with care. In addition to selecting a good brand the surface of the composite should be polished after placement to remove any uncured resin. Names like Bisco, and diamond crown come to minc.

Millions of people have tons of mercury in their teeth and it causes numerous problems. The removal of an old filling exposes everyone present to levels of mercury considered immediately hazardous to health. More realistic measurements of Hg vapor exposure are close to 20 microns/cubic meter per day for the average person in rush hour traffic. With amalgam contributing 1-5microns/cubic meter/day. Most believe that less than .01 % of Hg vapor is actually bio-integrated in the body. Mercury salts/inorganic mercury are far more dangerous. Yes saemons we must get rid of the other mercury products, but the ones we attach to your body and place in your mouth are ok. That is truly brilliant, I got mine removed and my health issues went away? The first week they got about three times worse and since have drastically improved. So the symptoms that the amalgams weren't causing got worse when I was exposed from the removal. Tha absesis that always were next to my fillings have not even once returned. As you probably know, there is polymerization shrinkage in all composites to varying degrees, just as there is expansion in amalgam. I believe I have seen enough cases of amalgam cracking teeth and composites getting recurrent caries to say that at least some of those instances were a result of the aforementioned phenomena, but not all.

I would agree that most dentists do not place composites correctly. Too many do not use rubber dam which is required for placing any composite restoration (as outlined by the manufacturer). In addition too many fail to realize that the bond strength is only to enamel NOT dentine or cementum. I don't know how many composites with decay I have seen because they used flowable resin in class II boxes that end on dentine/cementum. You NEED to do a two step method using a glass ionomer

There is no such thing as a "bacteria-free" margin. No filling material creates a seal with the tooth so that the margin is impenatrable to future bacteria and thus recurrent decay. When I began dentistry I called each patient the next day that i had I placed a 2 surface or more composite in: 153 patients, 2 with postoperative thermal sensitivity that resolved in 72 hours. In my opinion, most dentists just don't know how to properly place composites.

The reason we don't all just switch to the resin type is because they still aren't as effective in certain situations. Amalgam is stronger, it creates a seal (so that no bacteria can leak in between the restoration and the tooth), and they can be made bigger than resins (because they are stronger). Resins are polymers, which shrink when they polymerize, so that induces stress on the tooth, which may be painful. Also, they may shrink and get pulled away from the tooth, so bacteria leaks in.

Isn't this wonderful music? It's from a great album almost totally forgotten today and is in desperate need of reissue. As good as this track is you could argue it's not the best on the record. I just luckily stumbled onto a used copy of the US issue over 25 years ago and have been in love ever since. The reason some scandanavian countries have "banned" amalgam is because dentally they are possibley the most advanced countries and are therefore developing stronger tooth coloured filling materials (e.g.composites) which have in the past been nowhere near as good as amalgams in functioning as good restorative materials.
